Crews were instructed to clear the ship's hold in preparation for the next cargo. The ship operates with a slightly calm sea status without the ship moving.
To gain better access to the top area of the hold, the crew was instructed to place a high pressure wash system on a portable ladder in the tank.
The staging was approximately 6 metres tall and lashed to the ship’s structure to stay it stable. additionally , a security line was rigged across the hold to which the seaman on top of the staging could attach a security harness.
When it had been required to re-position the staging, the crew released the lashings and pushed the apparatus across top of the tank.
During this movement, the seaman on top of the staging lost his balance and fell down on to the tank top , had serious injuries. The vessel deviated and helicopter evacuation of the casualty.
